All "back to school" forms and documents can be found on this page. If you have any trouble viewing or downloading the forms that need to be returned, please contact the school office. If needed we can make arrangements for you to pick up paper copies.
The "Information Forms" contain important information that you will need to read. The forms marked with (*) require signatures on the "Signature Page" as confirmation that the information was read.
The "Forms to be Completed and Returned" need to be printed, filled out and returned to school. Some need to be completed for each child and some are one per family.
The "Optional Health Forms" include various medical forms for those students who have medical needs. Please fill out medical forms as needed for each child.
The "New Student Enrollment Forms" are for those Kindergarten students who are new to our school and have not been through the Kindergarten assessment.
The "Miscellaneous Forms" are for registration in the spring. This form will not be needed at this time.
All "Handbooks" can be found under the heading "Forms and Info". Please sign the "Signature Page" after reading the handbooks and explaining them to your students. The Diocesan Handbook is not available on this website. If you would like to view that handbook, please contact the school office and we can email or print a copy for you.
PRESCHOOL PARENTS: The only forms that you will need to read or fill out and return to school are marked with a (√ Preschool too). You are also required to read the Preschool Handbook and the Parent Student Handbook, and sign the Signature Page that you have read both handbooks. If you want to be included in the School Directory or your child has a medical issue, please fill out the Optional Forms that apply to your situation.
All forms need to be completed and returned to school on or before the first day of school, August 10, 2016.
